Kindergarten registration will open on February 1, 2022, to all residents of Upper Sandusky Schools. Children who will be 5 years old on or before August 1, 2022, are eligible to register and be screened.
You may register again this year via our school website. You can visit www.usevs.org and click on the link that says “Kindergarten Registration” and fill in the registration form. You will receive a follow-up call from the school to set up your appointment time for your child’s screening on April 21st or 22nd.
You can also call Linda Barth at the Superintendent’s office at 419-294-2306 between 8:30 am and 3:30 pm to register and set up an appointment for the screening. When you call, you will be asked to provide your child’s full name, address, phone number, and date of birth.
Approximately two weeks prior to the screening, parents/guardians will receive a letter confirming their appointment time. The deadline to register is March 31, 2022.
During the screening, your child will have an opportunity to meet the kindergarten teachers as well as participate in a screening for vision, hearing, general readiness levels, speech, and language. Kindergarten screening will take place on April 21 and April 22, 2022, at Trinity Evangelical Church, 108 Malabar Drive, Upper Sandusky. You will need to bring the following information about your child to the screening:
Birth certificate or other legal proof of age
Social security card
Proof of residency in Upper Sandusky school district
Current custody papers or guardianship papers (if applicable)
Immunization records:
Shots needed: DTaP - 5 doses, Polio - 4 doses, MMR - 2 doses, Hepatitis B - 3 doses, Varicella (chickenpox) - 2 doses
Please note: This is the only screening that will be held. You must register for this screening if you want your child to attend kindergarten for the 2022-2023 school year. If you have any questions, call Linda Barth at 419-294-2306.
