Kindergarten Registration for Upper Sandusky Exempted Village Schools
Kindergarten registration will open on January 25, 2024 to all residents of Upper Sandusky Schools. Children who will be 5 years old on or before August 1, 2024 are eligible to register and be screened.

You will need to sign up for a screening session on either April 4th or 5th. We have 4 sessions each day: 8:00 am, 9:45 am, 12:30 pm and 2:00 pm. These appointments are first come, first served, so sign up early to get your preferred screening time.
You can also call Linda Barth at the Superintendent’s office at 419-294-2306 between 7:30 am and 3:30 pm to register and set up an appointment for the screening. When you call, you will be asked your child’s full name and birth date, and parent/guardian’s name, address, and phone number..
Within a week of registering, parents/guardians will receive a letter confirming their appointment time. The deadline to register is March 24, 2024.
During the screening your child will have an opportunity to meet the kindergarten teachers as well as participate in a screening for vision, hearing, general readiness levels, speech and language. Kindergarten screening will take place on April 4 and April 5, 2024 at Trinity Evangelical Church, 108 Malabar Drive, Upper Sandusky. Copies of the following documents are required prior to your child’s screening:
- Birth certificate or other legal proof of age
- Proof of residency in Upper Sandusky school district
- Current custody papers or guardianship papers (if applicable)
- Immunization records:
- Shots needed: DTaP - 5 doses, Polio - 4 doses, MMR - 2 doses, Hepatitis B - 3 doses, Varicella (chickenpox) - 2 doses
In addition to the documents listed above, your child will need a physical that was done on or after September 1, 2023. Physical forms will be available at screening.
- If your child has had a physical within the year, take one of our physical forms to your doctor's office and have them fill it out.
- If your child has not had a physical yet, he/she must have one before starting school. Be sure to take one of our physical forms to the appointment.
If your child does not yet have all the required immunizations, you may call Wyandot County Public Health at 419-294-3852 to set up an appointment to get what is needed.
Please note: This is the only screening that will be held. You must register for this screening if you want your child to attend kindergarten for the 2024-2025 school year. If you have any questions, call Linda Barth at 419-294-2306.
ALSO NOTE: Our kindergarten screening is only for children that live in the Upper Sandusky School District, or students that have older siblings that have already been accepted into Open Enrollment. If you do not live in our district but are interested in attending, you can fill out an open-enrollment form during our April 1 - May 31 Open-Enrollment window. You can call 419-294-2306 after April 1st to request an application. Your child will need to be screened in your district of residence. If accepted into Open Enrollment, we will send for the screening results. Open Enrollment decisions are made in early June, pending class availability.
There will be no school for current kindergarten students on April 4 and April 5 to allow the kindergarten teachers to help with screening.
