Wyandot Memorial Hospital will be doing athletic physicals soon.
When:
- June 5-7 from 3 to 6 PM
- June 8 from 8 AM to Noon
Where:
- Wyandot Memorial Hospital
- 885 N. Sandusky Avenue, Upper Sandusky, Ohio 43351
Details:
- Athletes MUST be entering grades 7-12
- No walk-ins will be accepted. You must schedule an appointment to be seen.
- Final Forms are required to be completed and signed by the parent/guardian and brought to the appointment.
- A $20 Donation will go to the Athletic Department of the athlete's school
