Upper Sandusky High School and SportsCare at Wyandot Memorial Hospital are partnering to offer athletic physicals on Wed., May 29th at high school and middle school athletes (entering grades 7-12).
WMH staff and local chiropractors and optometrists will provide physical exams for a $15 donation to the USHS Athletic Department. The money raised will help purchase supplies for district teams.
The physicals will be held in the USHS gymnasium and no appointments are necessary. Athletes entering grades 7-9 will report from 5-6pm, and athletes entering grades 10-12 will report from 6-7pm.
Completed physical forms with parent signatures are required PRIOR to the exams by healthcare professionals. Student-athletes or their parents may pick up physical forms from the information rack outside the USHS main office or the middle school main office.
For added convenience, Wyandot County Public Health is offering vaccines during the physicals. Pre-registration for the vaccines by May 22nd IS REQUIRED to ensure an adequate supply of vaccine and obtain insurance and payment information.
