School Health Requirements
Parents, if your child will be entering kindergarten, 7th or 12th grade, please check the requirements below.
- All incoming kindergarteners are required to have a current physical and immunization record on file. Immunization requirements are as follows: 5 doses of DTaP, 4 doses of Polio, 2 doses of MMR, 3 doses of Hepatitis B, and 2 doses of Varicella (chickenpox) vaccine. Kindergarten vaccines can be given anytime after the child turns 4 years old.
- All incoming 7th graders are required to have a tetanus booster (Tdap) and meningitis (A, C, W and Y) vaccine. These vaccines can be given anytime after the child turns 11 years old.
- All incoming 12th graders are required to have a meningitis (A, C, W and Y) vaccine. This vaccine must be given after the child has turned 16 years old.
