Parents of 2018-2019 kindergarteners, 7th or 12th grade students!

Attn: Parents/Guardians (from school nurse Beth Bower)

If your child will be entering kindergarten, 7th or 12th grade for then 2018-2019 school year, please check the requirements below. If these requirements are not met, your child will not be able to start classes in August.

*All incoming kindergarteners are required to have a current physical and immunization record on file prior to the first day of school. Immunization requirements are as follows: 5 doses of DTap, 4 doses of Polio, 2 doses of MMR, 3 doses of Hepatitis B, and 2 doses of Varicella (chickenpox) vaccine. Kindergarten vaccines can be given anytime after the child turns 4 years old.
*All incoming 7th graders are required to have a tetanus booster (Tdap) and meningitis vaccine prior to picking up their class schedule in August. This vaccine can be given anytime after the child turns 11 years old.
*All incoming 12th graders are required to have a meningitis vaccine prior to schedule pick up in August. This vaccine must be given after the child has turned 16 years old.
